A couple of holidays, just before the lecture to co-op, an the extreme cable broadband reaches an agreement for us to acquire her customer base. Not necessarily the company, but the customer base. So we are now in the town of Swany. We've got about 85 poles to change out as part of our big building construction in the town of Swany. And as soon as we're done that, we'll get the fiber pulled and spliced. And then those customers that are there now taking service with extreme broadband, we'll be able to get much faster internet speeds at lower prices. So that means we're all aware of the poor customer. And so we decided to get in there and then taking care of that. As well, once we convert the very last extreme customer, then we'll reck out all of the existing extreme cable equipment and poles so Swany will get rid of all the additional poles that have been placed along the roads as well as some of the aerial cables so all that's heading in a great direction also a quick update on cross city you that may live in the city limits, we should immediately give you your polling file. At the tail end of the first quarter, or shortly thereafter, getting it spliced and a little more customers out here as well. We've had a little bit of a delay with getting all of the approvals from Duke Energy on the poll attachments, but that's coming a little quicker now. So that's the update. Yes sir, and also in my other roles with the University of Florida Mobile Outreach Clinic. We're a mobile clinic and once we get to fly voted up out, share with you and everyone here about what we've been doing with our rural outreach program over the last year and a half, and we are wanting to continue those services and hopefully even expand them beyond where we've currently been, which is And, Joker's in Levy. We've been providing services at Trenton and Lawton. Right, but the law don't necessarily do. Yeah, that's a lot of stuff. Let me. Let me just look at the road people there. Let me. Let me just go up the water. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. There. we've been doing and what we're capable of doing and hopefully some of our plans have been able to stand. This got on my radar even a little more based on the community health and community mix plan that was passed by the Tri-County area for this upcoming three-year term for bringing the new chip. So we were actually put on the chip, the mobile outreach clinic as a goal of increasing services. This wasn't because of me or anybody on my staff, this was based on community input and I suppose based on community community health assessment. So once I saw that I thought well I probably should figure out how to make a better effort to expand services. I'm going to the next slide. Yes, this is going to be there. Those are two vehicles so the one on my right is a bus which we we take only in a latch for a county. It doesn't get very good gas mileage. It isn't really a business. It's travel much outside of the county. But the bus on the left, we got through a grant from a program called Direct Relief about two years ago. And it's our world outreach vehicle. That vehicle is equipped within a complete patient care space so we can park in any partners parking lot, church, health department wherever it doesn't matter. And we can see patients in that unit do full physical advance, we can draw a last one of their services, we can do it over few years, the whole night. Go ahead and make a client And so just to let you know, so we're a three-click. So we, our services are designed to serve those that don't have insurance, and as people are probably aware, if you don't have insurance, medical care is extremely expensive. We obviously don't have department of health-based primary care anymore in the thriving area. That's a bit of a gap. So we started responding to that need, which we're mainly serving in the LATRA County, but me being a practitioner out here, I understood their pings, the need for more of those kind of services out here. Back then, I sent my own patients after they've lost insurance or their family members. You don have insurance back that patients wife on Friday, you know I said, come on out to my mobile clinic and train her once and we'll take care of you because she doesn't have insurance and it's able to afford well followed by for insurance. So that's our, that's who we're trying to, to sort of, and we understand that a lot of folks, especially folks that maybe have lower incomes and that's maybe one of the reasons they don't have insurance or an employed one where maybe there are a lot of other social challenges around that. I call them health related social needs and we really specialize in not only delivering full service primaries here but also providing the kinds of social service referrals, getting folks free medications, getting them connected with financial assistance programs, or UFLs and otherwise to make sure that we take care of as many needs as possible. Next slide. Okay, that's our full fleet. The exam room bottom, bottom middle, that's what the exam space in the Skurnder band is like. That's the one that we use for the rural average. This is the next one. I'm going to summarize this is what we do. Currently we're in most in the Latino Latino County just to give you an idea of what our kind of demographics are. It's typically both 18 to 64. We see a lot of demographic minorities, of course, folks about insurance, and almost all of our patients are less than 200% of their poverty, and actually the vast majority are at federal poverty level. And folks that otherwise wouldn't have access to medical care, you can skip this slide. So the stars are where our current rural outreach program has been. So we've been in Waldo, McIntyne, Opie, Bronson, and Trenton, but we're actually stopping our Waldo service and we switched our McIntyp service to quarterly and started increasing the amount of times we come out to the track down area because the need is so much greater. The demand is so much higher out here. And of course, I'm aware that the demand is significant in Dixie County as well. So what can we do? So we diagnose and treat acute and chronic conditions, so just like a few games have seen me at my clinic in Old Town, we can do just about all that stuff in our, in our vehicle. So we, we're trying to manage medications, we've got some testing, we provide research and health services to women. We can refer to diagnostic services, We can get three ultrasounds and gains vote through our program. We also are very good at getting people connected to programs that can provide free diagnostic services like nurse practitioners and other staff. I understand how to navigate all of those services. We can't portray any simple procedures. So we're originally funded for our operations by Catholic Charities. So we've got the vehicle through this order, this is called direct relief, but then Catholic Charities actually paid for our outreach or for our operational cost. Catholic Charities has pulled back a lot of their various outreach and rural areas and that included funding. So we're 85% grant funded despite having the name of University of Florida. We get very little money from the University of Florida. That's not for lack of trying. It's just the way it is. So we write a lot of grants and that makes it sometimes challenging to expand services. Was we want to make sure we're able to continue funding to things we already exist. So it's important for us to try and develop a new partnership. So I've been in front of all of the county commissions in the tri-county area, I've been reaching out to various potential donors in the area to try and figure out how we can keep this service going and, for my desire, expand it more into Dixie County, whether that means moving to Fanding old town or out here to cross city wherever it makes the most sense we would we would try to figure out how to do that Right now in the process of some grants became available because of the hurricane specifically for rural Based primary care services especially for folks impacted by the storm, folks that have financial difficulty. So we are applying for grants right now to try and hopefully extend our ability to provide service for the next year. That's only going to be 12 months. So I want to try and figure out how can we partner with folks out here, whether you local government or otherwise, to create some sort of sustainable plan for us being able to deliver services going forward? Because that's a... Now, I want to bring the board up to speed because I had to let him... Many of us have been talking, all Mark and my neighbor asked me to give us gentlemen a call. I did spend a month as an office. We went over on the office and stuff. I know they've been to talk in the legislators about them doing away with the solar water board that we put money into every year. German said that they could supply those services to this county for arrest and what we're paying for. So if something happens to to that and that would be a one way, the county taking that funding and moving it to them to help the people. And if we don't, if I don't go away, we still can try to figure out a way to come up with that through grants or donations, whatever, because what was the number that was in that? So it's roughly, so in rough figures, to fund all staffing and operations gas everything. Essentially a thousand dollars per clinic site for monthly services. So think about twelve thousand dollars for a year of monthly service to 60 counted with a Dixie County Day. So right now we have two gopers days, one leavey day. We were able to add a Dixie County Day so right now we have two Hilver State one-leab-day we made you know we were able to add a Dixie Day I mean quite well. I don't know how the resident commission is for you but I'm raised out of Dixie Day and we'll figure it out. I'm sure you're right. Yes sir. What's Harold? Yes sir. I've heard nothing but outsthing things about it and she will be excited to have a place to send the folks that she It's pretty much a call of on you. Need more advanced care. The ITC County is one of the counties selected for this fund in this year. I've attached a one page flyer about the point program. I passed that out to you earlier. We'll be setting up meetings with all the partners to discuss implementation. But I've already sent Chief Artsman, I'm not trying to tell you under the bus chief, an email regarding meeting to discuss this since the Parametizen program is such a key part of this effort and we know it takes time to get approval from the county. The fund is $825,000 for the first year, which is this year. The second year, the second year, we will get 412,500 and $206,250 for years, three through the end of the settlement. Now, she don't give me the end of the settlement. The dollars will carry forward if not fully extended each year. We will be looking at getting programs started as soon as we have the sign contract amendment from DCF. Since you mentioned you have a county commissioner making coming up, I wanted you to have the information. Please let us. Whether I have any questions, I'll put the email she sent to you first. And it's not working out too well to have a group meeting. So we're sketched on individual meetings with different county EMS groups. The six months for the permitting probably probably can't do nothing with that. It looks like it's going to take about six months to get the permit process. The FK's 90 days. But the engineering, the coastal engineering consultants that Locklear had hired had indicated in the meeting that they didn't think the budget was going to be sufficient to to finish 100% of the job. So is that the case? I don't mean personally. I would hate to see an incomplete job or a 75% job or 50% job. So if there's some funding shortfalls there and I would ask that we try to work together and see how we can come up with that. I know we got the hurricane legislation, appropriation programs coming up, the last funds, maybe the BP, just to make sure we got the funds in there to complete that job. And that may go into the swanning dredge as well. I just can't remember if he said that one would be a shortfall or not. It's possible. So the other thing was the roads in the county roads was inside a horseshoe. And again, I'm just going to ask, I know the CMS Department that handles the DI's, PWS, whatever they call it. Has that been infected? And do we have that on a list? 351's in pretty bad shape for most of the way and the pavers on the side.
