Department Head/Elected Official Signature: Date: - - Mc g - - we Funy 2/e/25 gar20e Contract No. 2025- CONTRACT FOR SERVICES (Independent Contractor) This contract for services entered into on this day of 2025, by and between the parties Comprehensive Family Care), a corporation authorized to do business in Georgia whose address is 2055 Gees Mills Road, Suite #315, Georgia, 30013 (hereinafter referred to as "Contractor"), and ROCKDALE COUNTY, GEORGIA, a political subdivision of the State of Georgia whose address is 962 Milstead Avenue, Conyers, Georgia 30012 (hereinafter referred to as "County"), sets forth the rights, understandings and obligations by and between Contractor and County for Rockdale County Family Treatment Court Program Evaluation. WITNESSETH: WHEREAS the County, by and through Rockdale County, has been awarded State and/or Federal grant funding; and WHEREAS the Contractor desires to perform the services as required by State and/or Federal grant funding as set forth in Exhibit "A"; and WHEREAS the County, in order to fulfill the terms and conditions of State and/or Federal grant funding guidelines desires that Contractor perform the services as required by aforementioned funding; and ITIS HEREBY MUTUALLY AGREED AS FOLLOWS: 1. Contractor shall perform, for Rockdale County, Georgia, Strengthening Families programming for the Rockdale County Family Treatment Court as set forth in Exhibit "A". Contractor shall perform said services within the parameters established by Rockdale County, independent of any day-to-day operational instructions from the County. Contractor shall submit a monthly report to the applicable Rockdale County department or courts as to work performed under this contract, itemizing the number of hours worked during thei month. 2. The County shall pay Contractor an amount not to exceed $9,900 for services provided to Rockdale County. Contractor shall be paid for services rendered as set forth in Exhibit "A" for Strengthening Families programming for the Rockdale County Family Treatment Court. The Contractor shall submit to Rockdale County a monthly statement showing the dates and times worked, a brief description oft the services provided, the total number of hours worked, the hourly rate of payment and the total amount due. The rate of payment for any services shall not exceed the amount prescribed in this Agreement for provision of services between Rockdale County and Contractor. Payments will be made monthly according to the accounts payable schedule maintained by the Finance Department. Rockdale County reserves the right to suspend payments should the Contractor fail to provide the required reports in a timely and adequate fashion or ifContractor fails to meet other terms and conditions of the Agreement. Contract No. 2025- Contractor acknowledges that this Agreement involves the use of federal funds and as such, is subject to audit by the agency oft the United States Government granting the funds to the County for the purposes of performing the work and activities as stated in Exhibit "A". Contractor shall fully reimburse the County for any cost of the Contractor which is disallowed by any federal agency and which must be refunded thereto by the County. 3. Contractor acknowledges that he/she is not an employee of the County and that said relationship between Contractor and the County can be severed at any time. Contractor acknowledges that he/she is an independent Contractor with the County and consequently Contractor shall be responsible for the procurement of his/her own health, liability, and workers' compensation insurance policies. Contractor will carry sufficient insurance coverage to protect contract assets from loss due to theft, fraud and/or undue physical damage, and at a minimum will purchase al blanket fidelity bond covering all employees in an amount equal to cash advances from the County. Contractor will provide Statutory Workers' Compensation insurance coverage for all its employees involved in the performance ofthis Agreement. In addition, Contractor shall be responsible for contributing payments for, as provided by law, all FICA, withholding and all other taxes, to the proper agencies oft the United States and State of Georgia in connection with the compensation provided by the County in conjunction with the services rendered pursuant to the terms and conditions of the State and/or Federal grant funding. 4. Services that cannot be provided by Contractor shall be sub-contracted with a qualified provider. Contractor shall include expenses in monthly invoice and Contractor shall be responsible for paying Sub- Contractor. 5. Any termination ofthe State and/or Federal grant funding to Rockdale County shall result in a total and immediate termination of this contract between the County and Contractor. Contractor hereby acknowiedges that all funding, resources, and expenses in connection with this contract are exclusively controlled by the provisions oft the aforementioned State and/or Federal grant funding. Any termination of funding providing for resources and expenses to the County shall result in an immediate termination of this contract and the County shall not be responsible for the funding, providing of resources or expenses in regard to any position held by Contractor. This contract shall not be modified by either the County or Contractor except in writing signed by both parties hereto. 6. This contract is effective 2025 and shall continue through September 30, 2025, and is renewable for two (2) additional one-year terms, renewable each year. Year 2 - October 1, 2025 through September 30, 2026. Year 3 - October 1, 2026 through September 30, 2027. 2 Contract No. 2025- 7. Notices required by this Agreement shall be made in writing and delivered via mail (postage prepaid), commercial courier, or personal delivery or sent by facsimile or other electronic means (provided that receipt is confirmed). Baig, County Attorney 4 Contract No. 2025- EXHIBIT A SCOPE OF WORK Strengthening Families (SFP) is an evidence-based family skills training program for high-risk and general population families that is recognized both nationally and internationally. During the course ofthe 14-session program, parents and youth (age 3 and older) attend weekly SFP skills classes together, learning parenting skills and youth life and refusal skills. They have separate class training for parents and youth the first hour, followed by a joint family practice session the second hour. SFP teaches, and has parents and youth practice, skills involved in bonding (creating warm, loving relationships), setting clear, firm boundaries (rules against antisocial behavior, including drug and alcohol use), and monitoring their children's emotional well-being and activities to see that they always stay in an alcohol and drug-free social environment. Skill practice creates new prosocial habit patterns in the brain, which helps improve behavior, strengthens the parent-child relationship, and helps a child feel loved. Child maltreatment decreases as parents learn better parenting skills, and practice stress and anger management techniques. Service Delivery: All SFP therapists, supervisors and facilitators will receive required initial and ongoing training on the model from the respective national dissemination organization to ensure model adherence. The Contractor will broker this training for all selected façilitators and engage in additional readiness, technical assistance, and support activities to ensure facilitator effectiveness. The Contractor will monitor model fidelity and will provide ongoing training and technical assistance to address improvement needs. SFP groups will be facilitated by the Contractor according to a calendar agreed upon with the Family Treatment Court Coordinator and in accordance with participant schedules. Program referrals will be sent to the Contractor by the Family Treatment Court Coordinator. The Contractor will supply an electronic platform and login information to conduct virtual sessions and/or the space needed to deliver the program in-person. The Family Treatment Court Coordinator will supply all meals needed to ensure compliance with SFP. The Family Treatment Court Coordinator will supply all office supplies needed for participation in group sessions and/or needed by facilitators for program delivery.
