DEPARTMENT OF PUBLIC HEALTH  MINUTES SEPTEMBER 25, 2024  Call to Order:  Cory Wall moved to call to order the Board of Public Health meeting at 1:00 PM, seconded by Stan Hitchcock. The motion passed by unanimous vote by Dave Hornung. Those present were Cory Wall, Dave Hornung, Stan Hitchcock, Theresa Korbelik, and Nancy Gilley. Valerie Boyd was absent. 1.A FY25 Nurse Family Partnership Contract Amendment - Ends 12-31-2024-5162,255  1. Action Items:  - Public Health & Environment (054) Nurse Family Partnership  Stan Hitchcock moved to authorize the digital (DocuSign) signature for the Nurse Family Partnership (NFP) Contract -( CT QAAA2025-1076. Aml; GAE QAAA 2025-184 for July 1,2024, through December 31, 2024, with a grant award of $162,255 noting that this is the final contract for this program, seconded by Cory Wall. The motion passed by  unanimous vote by Dave Hornung.  2. Purchase Orders:  2.A PO#24-00556 - R&S Northeast - Contraceptives-S569.86-PH&E  (048) Family Planning  Cory Wall moved to approve Purchase Order #24-00556 for the Public Health & Environment PO#4677 to R&S Northeast for contraceptives totaling $569.86, seconded by Stan Hitchcock. The motion passed. Dave Hornung voted against the motion. 2.B PO#24-00557- - Glaxosmithkline Immunizations $2,881.20-PH&E  (032) Immunizations  Stan Hitchcock moved to approve Purchase Order #24- 00557 for the Public Health &1 Environment PO#4678 to GSK fori immunizations totaling $3,031.20, seconded by Cory  Wall.
