PCCD requires a signature page to request an extension. The request includes authorization for the Chairman to sign. B. Facilities & Operations 1. Approval to execute the option to renew the Agreement with Iron Mountain for shredding and storage for an additional two years (7/1/24 - 6/30/26) as envisioned by the current agreement, with a 3% increase each July. 2. Approval of an Agreement with' WGL (natural gas) requesting authority to execute the renewal agreement that will start upon expiration of the current contract (1/2025) at a rate not to exceed $4.473 per Dekatherm, for the 18-month period 1/2025- 3. Approval of an Agreement with FREEPOINT (electric) requesting authority to execute the renewal agreement that will start upon expiration of the current contract (1/2025) at a rate not to exceed $0.0767 per Kilowatt-hours, for the 12-month period 6/2026. 1/2025- -1 12/2025. Chestnut, Slippery Rock and SouthEast). CoPilot Annual Home Delivered Meals Software Subscription (no rate increase): 250-500 consumers - $500/month; 500-750 consumers CoPilot Pro21 Software Subscription (increase $75/month): $1,175/month. Consultant Agreement (increase $10/hour): $130/hour. $750/month. Each of these subscriptions are needed to import specific data into the State's SAMS database for the PA Department of Aging to review. The agreement term is 7/1/24- 6/30/25 for the Senior Centers and Home Delivered Meals subscriptions. The CoPilot Pro 21 subscription and Consultant Agreement term begins 7/1/24 with no end date (30 day termination clause contract can be terminated by either party). There are no County funds involved; payment will be made from the Aging 2. Approval of the renewal of the Home Support Services and Personal Care Services contracts for the period 7/1/24 - 6/30/27, with the providers listed below. There are no County funds involved; payment will be made from the Aging Block Grant; there is Block Grant. no rate increase: Anova Health Care Services, Inc. Cambridge Home Health Care, Inc./Private dab Cambridge Home Health Care Community Resources for Independence Helping Hands Home Care Services, Inc. The rates for each contract are as follows: Personal Care Contract: $27.60/hour Home Support Contract: $27.16/hour- Basic $30.93/hour - Intermediate 3. Approval to enter into a Professional Agreement with Jennifer Antkowiak, Founder of Take Care Tips, to speak at the Butler County Area Agency on Aging's Caregiver Event on 6/26/24. The Speaking Fee is $1,000; this event has been approved by the PAI Department of Aging; there are no County funds involved; payment will be made from the Aging Caregiver Funding. G. Parks and Recreation 1. Approval to update and re-record the Alameda Park Deed for the Land and Water Conservation Fund (LWCF) deed restriction and the official LWCF Boundary Map. The deed has been reviewed, approved and met the requirements of DCNR. This update is required by DCNR and LWCF for park property that received 2. Approval of Resolution No. 2024-9, requesting a Greenways, Trails and Recreation Program (GTRP) grant in the amount of $250,000 from the Commonwealth Financing Authority to be used for the Diamond Park Rehabilitation and LWCF funding. Improvement project. H. Planning 1, Approval of a renewal Consulting Agreement with Delta Development Group, Inc. for a24-month period through 6/2026. The monthly compensation of $12,500 will remain the same. Planning - Food Program 1. Approval of assignment of the Butler County Food Program to Greater Pittsburgh Community Food Bank of Duquesne, PA. This organization already works within Butler County handling the County's senior box food program and handles food distribution services for ten other counties in Southwestern Pennsylvania. Under this contract they would be handling the County's TEFAP (The Emergency Food Assistance Program) and SFPP (State Food Purchase Program). Services would 2. Termination of the Service Agreement by and between the County of Butler and Community Partnership, Inc. of Butler, PA for the Butler County Food Program (TEFAP and SFPP); said termination of both programs to be effective as of 6/30/24. (There is a form to be completed and sent to the State.) 3. Termination of a space Lease Agreement with Community Partnership, Inc. for the warehouse at Building #9 to be effective as of 6/30/24. 4. Termination of Equipment Use Agreement currently held between Community Partnership, Inc. and County of Butler for various food equipment owned by the County of Butler Food Program. This Lease was in effect until such time as the Community Partnership, Inc. no longer manages the Butler County Food Program. As such, this Equipment Use Agreement is hereby terminated as of 5. Approval of a Service Agreement by and between the County of Butler and Greater Pittsburgh Community Food Bank for the operation of the SFPP Food Program effective 7/1/24 through 6/30/25 (details regarding allocation are not yet finalized); said action is contingent upon Solicitor's approval of the final Agreement. 6. Approval of a Service Agreement by and between the County of Butler and Greater Pittsburgh Community Food Bank for the operation of the TEFAP Food Program effective 7/1/24 through 9/30/25 (details regarding allocation are not yet finalized); said action is contingent upon Solicitor's approval of the final Agreement. 7. Modification of the current Memorandum of Understanding for space at County Building #8 a/kla the Grainery, which is currently being used for the administration of the Food Bank Program. This Agreement will be modified to reflect that Community Partnership, Inc. will be able to use the space for administration of other state and federal programs, including, but not limited to, CSBG and other social services programs. All other terms and conditions of the original Memorandum of begin 7/1/24. 6/30/24. Understanding will remain in effect. J. Planning Recycling & Waste Management 1. Ratification of a memorandum regarding a grant application applied for on behalf of the Blind Association, approved at the 5/8/24 public meeting, for one box truck for the Out of Sight Shredding business. After the public meeting, we were made aware that they were in need of two box trucks and a memo was circulated on 5/9/24 requesting this change. There is still no cost to the County other than administrative time as 90% of the purchase price, if the grant is approved, will be refunded through Act 101, Section 902, Recycling Equipment Grant and the Blind Association will pay the required 10% match to be outlined in the Cooperation Agreement. K. Property and Revenue 1. Approval of settlement of Luellen S. Henderson assessment appeal; Case No. 2023- 40343. L. Commissioners 1. Brandon Savochka requested approval of five Agreements with. Avanco International, Inc. related to the Child Accounting and Profile System Application (CAP-C CYS data management system) for the period of 7/1/24 - 6/30/25: / Authorized User 24/7 access (up to 65 users) in the amount of $40,185.65. Butler County's share for the development of the CWIS 2024/2025 in the amount of $4,462.97. Upgrades, which include changes necessary to keep the system in compliance with the federal and state mandates for reporting. Butler County is 10f56 counties sharing in the cost of the upgrades, with authorization for the Chairman to sign. HIPAA Business Associate Agreement. Part of the approval, with authorization for the AFCARS Upgrade and Maintenance totaling $3,607.66, with authorization for the Consulting contract not to exceed $50,000. Chairman to sign. The contractor was contacted and admitted that he did not do this even though the requirement and directions were available in the bid package. According to DCED's CDBG rules and regulations, we are required to throw out the bid as "invalid". Upon further review, we found that the second-lowest bidder, Holbein, Inc., did submit all the required federal documents and all the paperwork were found to be in order. Upon the recommendation of the engineer, Widmer Engineering, we are asking approval to award to Holbein, Inc. int the amount of $102,486.10 for Base Bid, Alt 1 and Alt 2. This will be funded using 2022 CDBG Commissioner Geyer made the motion; seconded by Commissioner Boozel. The motion carried 1. Sheryl Kelly requested approval to award a contract for consulting services for the update of the County's Municipal Solid Waste Management Plan as required by Act 101. These services initially included the solicitation of proposals from landfills interested in providing guaranteed landfill space for municipal solid waste produced within Butler County for the next decade. Proposals were due Friday, April 19, 2024. All three consultants that submitted proposals have indicated that they have no objections to any of the terms int the agreement. This project is eligible for an 80% funding match through Act 101, Section 901 Planning Grant from DEP. The three proposals received are as follows: MSW Consultants, Orlando, FL (with an office in Cumberland County, PA)-$68,600 Ms. Kelly requested to award the contract to Nestor Resources, Inc., Valencia, PAI in the Commissioner Geyer made the motion; seconded by Commissioner Boozel. Sheryl Kelly requested approval to submit a grant application, if needed, int the name of Butler County on behalf of the Blind Association for the purchase of a box truck for their Out of Sight shredding business. The grant is Act 101, Section 902, Recycling Equipment Grant and under the terms, only municipalities can apply, and they must maintain ownership of any funded equipment. In this case, the County would maintain ownership of the truck, but the Blind Association would be responsible for all insurance, liability, registration, inspections and maintenance of the truck. The Blind Association is ini the process of contacting two local municipalities about their possible participation, but if that falls through, the County would be the applicant, if approved. There is a 10% match on this grant that the Blind. Association will be responsible for, so there is no cost toi the County other than administrative time. unanimously. D.
