July 1, 2023 through June 30, 2024. Purpose of contract: To continue to provide the Ride Program. Will be reimbursed $1.50 per mile and 5% administrative fee. Item 2: Everyfit dba QMedic AAA Synopsis: Requests Commissioners approval for ratification of a new contract with Everyfit dba QMedic for the period of January 1, 2024 through June 30, 2024. Purpose of contract: Provide the following services to the older individuals as authorized by the County: In-Home PERS-$30 per client per month; Mobile GPS PERS- $30 per client per month; Mobile GPS PERS Plus- $30 per client per month; Mobile GPS PERSX- $30 per client per month; Mobile GPS PERS wristband- $30 per client per month. Item 3: Oil Region Recovery SUD Synopsis: Request Commissioners approval to amend and existing contract with Oil Region Recovery for the period of. July 1, 2023 through June 30, 2024. Purpose of contract: Provider to add outpatient services at an individual rate of $35.75/15 minute unit and group rate of $10.27/15 minute unit; also, intensive outpatient services at a rate of $10.87/15 minute unit for both individual and group proposing these rates to be retroactive to 12/20/2023. Item 4: Clearbrook Treatment Center, LLC dba Huntington Creek Recovery SUD Synopsis: Request Commissioners approval to enter into a new contract with Clearbrook Treatment Center, LLC dba Huntington Creek Recovery Center for the period of. January 1, 2024 through June 30, 2024. Purpose of contract: To provide adolescent detox at $579.00/day; adolescent non-hospital short term residential treatment at $472.00/day; adult detox at $441.00/day; adult non-hospital short term residential treatment at 425.00/day Item 5: Seniors Farmers Market Nutrition Program AAA Synopsis: Requests Commissioners approval to amend an existing contract with Seniors Farmers Market Nutrition Program. For the distribution of SFMNP checks on the department's behalf to eligible senior citizens and now wish to amend the terms of agreement to account for changes in program administration required by Federal regulations.
